The information on the following pages reflect the results of National Health Information Network's (NHIN) testing of providers, software vendors and third party claims processors (test participants) for pharmacy claims for the handling of claims in the NCPDP 5.1 standard as defined in SNIP 1A and SNIP 1A Plus. Effort has been made to make the information compiled as accurate and complete as possible to the best of NHIN's knowledge. Any questions or possible corrections to the information should be directed to the NHIN 5.1 project manager at nhin5.1@nhin.com. Successful completion of the test means that the respective test participants were able to submit/process claims in the NCPDP 5.1 standard format as described. NHIN cannot and does not provide any warranties for the test participants and also does not warrant that any of the test participants so tested are free of any other problems associated with claim submission/processing. FOR THESE REASONS, NHIN DOES NOT PROVIDE ANY WARRANTIES, EXPRESSED OR IMPLIED, INCLUDING FITNESS FOR A PARTICULAR PURPOSE, MERCHANTABILITY OR WORKMAN LIKE MANNER. The testing performed by NHIN of the test participant's submission/processing of claims was conducted with the PDX Pharmacy System or PDX Adjudication Simulator which formats the claim in the NCPDP 5.1 standard format. Subsequently, each provider/software vendor tested claim submissions with an NHIN assigned pharmacy claims processor.

I have read, understand, and agree to the above, and wish to continue.
